A second set of clamps provides additional stability for going up and down curbs and steep ramps. Most wheelchairs have a push handle height of only 36” off the ground. For anyone who is average height or above, pushing a wheelchair with such low handles can be a problem. If you (or your caregiver) are taller, then you will understand how uncomfortable it can be to stoop over to push a wheelchair. Also, you could be inviting back strain, neck strain and poor posture. Nice, but needs modification by user to ensure workability.
I am 6'3" and pushing a wheelchair is a pain in my back. I bought these in hopes of being able to push chair while fulling standing up. They do work fine for that. However, The continually move around in the brackets. I have it as tight as I can make it and they still move when pushing. I had to drill a hole in the bracked and wheelchair handle to screw in a small sheetmetal screw to prevent it form spinning/sliding. This fixed that problem. However, now they slide arounf in the other side fo teh clasp. Will have to drill another hole in bracket and wheelchair again to stop the spinning. Close to throwing them across the street in anger. Hopefully with another set of holes drilled and screws tighted it will function well. We shall see.

Terrific product, but get a grip
Once you get the hang of installing the handle extenders, these things are fantastic (get the heavy duty version, which comes with two extender braces for each side). They’re well built, and again after you’ve fiddled enough with the installation, they’ll be very firmly in place while being easy to remove if you need to stow the wheelchair in, say, the trunk of the car.That said, the grips are thin vinyl things that will slide off in your hand precisely when you need a firm, dependable grip on the wheelchair. This is easily addressed by replacing them with something like mountain bike grips (I had great success with https://smile.amazon.com/gp/product/B00FN9U3TQ/ref=ppx_yo_dt_b_search_asin_title?ie=UTF8&psc=1). You may need to rub on a little bar soap or put a tiny drop or two of dishwashing detergent to get them on the extender handles, but once those things are in place (and the soap has dried), you’ll be amazed at how well this system works.

Helped stop the backache
My husband is 6'2" so ends up with a backache every time we are out for any length of time. We looked for some kind of way extensions to help raise the handles up. After trying them out on a weekend trip, these worked out the way he had hoped. Even while on the grass.

Without the extra clamps which are way overpriced they are useless. However
I would have given 5 stars if the extra clamps were included or if they made a longer clamp. Without the extra clamps which are way overpriced they are useless. However, with the extra clamps they are a back saver. One clamp (at least as designed) does not hold the handles in place.

Ordered two, both regular and heavy duty. Both arrived looking the same. Not sure what the difference is or even which is which

Nice addition on the wheelchair
Good idea, good quality and good relief for the back when pushing the wheelchair. I was able to install these but as suggested, I would order the additional clamps for the bottom of the extenders to prevent the extenders from moving out of position. Not a huge problem but it does sometimes happen when loading and unloading the chair and also during extensive use.

Helps my back to stay upright. Makes it easy to speed along with my rollstor.

They work very well on a walker also.
rubber/plastic Handel's are a little cheap. Even if it is an extra sale they should make them quality handles/grips available.
